What do you get with your Membership Plan?
- Dental Cleanings: Two or three oral exams and professional dental cleanings.
- X-rays (as needed): One set of check-up (including bite-wings) X-rays annually.
- Preventive Care: Two or three complimentary fluoride treatments at time of cleanings.
- 20% Discount on Restorative Services: Fillings, crowns, implants, dentures, and more.
- Priority Scheduling: Access to convenient appointment times and emergency appts.
How much does it cost? It depends on your prescribed course of hygiene treatment:
- Prophy (healthy mouth) Plan: Patient cost is $475 - covers two cleanings and exams.
- Perio (gum disease) Plan: Patient cost is $695 - covers three cleanings and exams.
Terms and Conditions
- Membership valid for 12 months from the date of enrollment.
- Enrollment fee is non-transferable and non-refundable and cannot be canceled or “carried over” to another 12-month period.
- Membership fee due at the time of enrollment. Treatment fees are due at the time of service and are separate from the membership plan fees
(exception is included services).
- Discounts are calculated using our standard fee and cannot be combined with insurance.
- Plan covers hygiene and restorative care only. Excluded treatments in the plan are TMJ/Sleep appliances, whitening, clear aligners,
orthodontic services, veneers, and Botox.
